Probiotics for Infantile Colic: Randomized Controlled Trial of L. reuteri

This double-blind RCT enrolled 400 breastfed infants with colic to evaluate Lactobacillus reuteri supplementation. Infants receiving the probiotic showed significantly reduced crying time compared to placebo, with effects observed within 7 days of treatment initiation.

Key findings: (1) Mean crying time reduced by 51 minutes/day at 21 days; (2) 73% of treatment group had crying <3 hours/day vs 36% placebo; (3) No adverse events reported. Effect was specific to breastfed infants.

L. reuteri may be considered for breastfed infants with colic after excluding other causes. This is one of several evidence-based interventions alongside feeding adjustments, carrying techniques, and parental support.

Wait 30 minutes after eating before swimming

What the evidence shows: There's no evidence that swimming after eating causes cramps or drowning. While digestion diverts some blood flow to the gut, it doesn't impair swimming ability in normal circumstances. Common-sense supervision is always important regardless of meal timing.

Warm milk helps children sleep

What the evidence shows: While milk contains tryptophan (a sleep-promoting amino acid), the amount is modest. Any sleep benefit likely comes from the comforting ritual and warmth rather than pharmacologic effect. A consistent bedtime routine matters more than any single component.

Cracking knuckles causes arthritis

What the evidence shows: Studies comparing habitual knuckle-crackers to non-crackers found no increased arthritis risk. The cracking sound is from gas bubble release in synovial fluid. The main consequence may be reduced grip strength and annoyed family members.

Early Pet Exposure and Immune Development: Birth Cohort Study

This birth cohort followed 1,200 children from birth to age 5, comparing immune development and allergy rates in homes with and without pets. Early pet exposure was associated with reduced allergy risk and altered gut microbiome.

Key findings: (1) 30% lower allergic sensitization with dog exposure in first year; (2) Cat exposure showed similar but smaller effects; (3) Microbiome diversity increased in pet-exposed children.

Reassure families that pet exposure during infancy may actually protect against allergies. For families without pets, don't specifically recommend getting one, but dispel the myth that pets must be removed to prevent allergies.

CFTR Modulator Therapy in Young Children: Expansion to Additional Mutations

New clinical trial data supports expanding CFTR modulator therapy to children as young as 1 year with various CFTR mutations. Early treatment initiation shows benefits in lung function preservation and nutritional status, with excellent safety profile.

Key findings: (1) 85% of CF patients now eligible for modulator therapy; (2) Early treatment prevents rather than reverses lung damage; (3) Growth parameters improved in treated toddlers.

Refer CF patients promptly for genetic classification and modulator eligibility assessment. The treatment landscape has transformed—many children can now expect near-normal life expectancy with appropriate therapy.
