Iron Deficiency in Infancy and Long-Term Cognitive Outcomes: 10-Year Follow-Up

This prospective cohort followed children with iron deficiency anemia in infancy through age 10, comparing cognitive outcomes to iron-sufficient peers. Despite iron repletion in infancy, children with early anemia showed persistent differences in executive function and spatial memory.

Key findings: (1) Early iron deficiency associated with lower scores on math and reading assessments; (2) Differences in brain structure observed on MRI at age 10; (3) Duration of deficiency correlated with magnitude of effect.

Emphasize prevention through dietary counseling and appropriate screening. Iron-rich complementary foods should be introduced at 6 months. Consider iron supplementation in high-risk populations.

Popular Beliefs

Warm milk helps children sleep

What the evidence shows: While milk contains tryptophan (a sleep-promoting amino acid), the amount is modest. Any sleep benefit likely comes from the comforting ritual and warmth rather than pharmacologic effect. A consistent bedtime routine matters more than any single component.

Sugar makes children hyperactive

What the evidence shows: Multiple double-blind, placebo-controlled studies have found no significant effect of sugar on behavior or cognitive performance in children, even in those reportedly 'sensitive' to sugar. The perceived hyperactivity is often due to the context (parties, holidays) or parental expectations. However, limiting added sugars remains important for dental health and nutrition.

Eating carrots dramatically improves vision

What the evidence shows: While carrots contain vitamin A important for eye health, eating extra carrots won't improve normal vision or eliminate the need for glasses. Vitamin A deficiency can cause vision problems, but this is rare in developed countries with varied diets.

Early Pet Exposure and Immune Development: Birth Cohort Study

This birth cohort followed 1,200 children from birth to age 5, comparing immune development and allergy rates in homes with and without pets. Early pet exposure was associated with reduced allergy risk and altered gut microbiome.

Key findings: (1) 30% lower allergic sensitization with dog exposure in first year; (2) Cat exposure showed similar but smaller effects; (3) Microbiome diversity increased in pet-exposed children.

Reassure families that pet exposure during infancy may actually protect against allergies. For families without pets, don't specifically recommend getting one, but dispel the myth that pets must be removed to prevent allergies.

Familial Hypercholesterolemia Screening in Childhood: Cascade Screening Effectiveness

Cascade screening for familial hypercholesterolemia starting in childhood identifies affected relatives and enables early statin therapy. Children identified through family screening had better outcomes than those diagnosed after cardiac events.

Key findings: (1) Each identified case led to 3 additional diagnoses in relatives; (2) Early statin therapy (age 8+) reduced lifetime cardiovascular risk; (3) Universal lipid screening at ages 9-11 complementary to cascade screening.

Take family history of premature heart disease seriously. Screen children in affected families by age 2. Universal lipid screening should occur at 9-11 years per AAP guidelines.
